Transient Trifascicular Block in Severe Hyperkalemia.

چکیده
Hyperkalemia is a commonly encountered electrolyte abnormality that can significantly alter normal cardiac conduction. Potentially lethal dysrhythmias associated with hyperkalemia include complete heart block and Mobitz Type II second-degree AV block. We report a case of trifascicular block, due to hyperkalemia. The patient's symptoms and electrocardiogram (ECG) evidence of trifascicular block resolved with lowering of serum potassium levels, with subsequent ECG showing left anterior hemiblock. This paper highlights an infrequently reported dysrhythmia associated with hyperkalemia that emergency physicians should be familiar with.

Transient Left Bundle Branch Block due to Severe Hyperkalemia
Hyperkalemia is a potentially life-threatening electrolyte imbalance that can lead to sudden death from cardiac arrhythmias and asystole. We present a case of transient left bundle branch block pattern on an electrocardiogram (ECG) secondary to hyperkalemia in a patient with history of end-stage renal disease. A 52-year-old man presented to the emergency room (ER) with chief complaints of weakn...
متن کاملIncidence and site of atrioventricular block in patients with chronic bifascicular block.
Four hundred fifty-two patients with chronic bifascicular block and initially intact atrioventricular (AV) conduction were detected, studied, and prospectively followed between January 1970 and March 1978. There were 360 males and 92 females, ages 18--93 years (mean +/- SD, 62 +/- 15 years). Follow-up ranged from 29-2804 days (mean 1066 +/- 97 days). AV block (2 degrees or 3 degrees) developed ...
متن کاملA rare form of trifascicular block with intermittent complete atrioventricular block in a patient with Chagas disease.
Trifascicular block, which consists of impaired conduction in the three main fascicles of the ventricular conduction system, may progress to high-grade or complete atrioventricular block. Exceptionally, it is possible to register in the same patient paroxysmal alternating atrioventricular block and bilateral bundle branch block. This is the electrocardiogram of a male, 60 year-old patient comin...
متن کاملSymptomatic Trifascicular Block in Steinert's Disease: Is It Too Soon for a Pacemaker?
We report a case of a 62-year-old male with Steinert's disease who presented with progressive intermittent episodes of lightheadedness five years after he was diagnosed with the disease. On evaluation, he developed a new onset trifascicular block (first degree atrioventricular block, new onset right bundle branch block, and left anterior fascicular block). A dual chamber pacemaker was inserted ...
متن کاملIntraventricular trifascicular block. Observations on conduction disturbance in bundle-branch system.
A case of intraventricular trifascicular block secondary to an inferior myocardial infarction is reported. The case denonstrates instances of 2: I atrioventricular conduction due to alternating block in the right bundle-branch and block in the right bundle and both anterior and posterior divisions of the left bundle (trifascicular block). Instances of I: i AV conduction with grade I block and p...
